[New York] The Client Assistance Program (CAP) provides support and advocacy for individuals with disabilities who are seeking vocational rehabilitation services from VESID (Vocational and Educational Services for Individuals with Disabilities) or CBVH (Commission for the Blind and Visually Handicapped). CAP offers information and assistance with gaining access to appropriate rehabilitation services in New York State. Advocates from the Client Assistance Program provide aid and strategies for negotiating with VESID, CBVH, ILC's, and related service systems. CAP is committed to promoting consumer access to quality services and supports. The Western New York Client Assistance Program (WNY CAP) is a free service and serves consumers residing in: Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, and Wyoming counties. As part of a statewide network of advocates, the CAP program is administered by the New York State Commission on Quality of Care (NYSCQC), and operates independent of VESID or CBVH. [Oregon] The Modest Means program matches low income Oregonians with attorneys willing to work for reduced fees. Attorneys that participate have agreed to charge no more than $35 for the first consultation, a reduced retainer, and no more than $60 per hour to Modest Means Clients. Clients must be able to pay for any services. Modest Means attorneys only handle landlord/tenant, family law, and criminal defense. There are a limited number of Modest Means attorneys, and they are not available in all areas of Oregon. They help families take the guesswork out of choosing care by providing referrals, information on state licensing requirements, availability of child care subsidies, and other pertinent information. CCR&Rs provide this personalized guidance by phone, in person, and over the Internet. They work cooperatively with all types of child care programs/providers, and do not discriminate in referrals based on race, sex, age, or national origin. [Colorado] Burial Assistance Information The Denver Department of Human Services Burial Assistance Program provides financial assistance for Denver county decedents who do not leave sufficient funds for their funeral and burial expenses, and whose family is either financially unable to provide for such expenses or cannot be contacted within a reasonable time. The total cost of the funeral and burial cannot exceed $2500.00. Applicants for burial assistance must qualify financially. The assets and resources of the deceased and all responsible parties will be considered when determining eligibility. The Burial Assistance Unit is open from 7:30 a.m. until 4:00 p.m. Monday through Friday. Applicants are seen by appointment only. To make an appointment, call Cherie Madden at 720-944-2175. Please have the following information ready when you call: * decedent's social security number * decedent's date of birth * decedent's date of death * name of the desired mortuary and/or cemetery Most mortuaries participate in our program; only a few cemeteries do. In most cases, an appointment will be scheduled for the same day. Eligibility will usually be determined within 24 hours. If you are a health care provider and you need the name of the next mortuary on the list, please call the Rotation Hotline at 720-944-3000.
